Coliseum Medical Centers, in partnership with Amerimed, can provide house-calls using physicians, nurse practitioners and paramedics. Their goal will be to help keep your recovery on track and to reduce avoidable hospital readmissions.
Who may benefit?
Patients that suffer from chronic illnesses or comorbidities. The most common are:
- Chronic Obstructive Pulmonary Disease (COPD)
- Congestive Heart Failure (CHF)

Does my insurance cover them?
House-call fees are based on usual office visit fees and are covered by Medicare and most health insurance.
Does this service replace your doctor?
This service does not replace your doctor. You will still need to go to your primary care physician and/or specialist for a follow-up visit. Our visit is designed to be a bridge between your hospital discharge and your follow-up doctor’s visit.
Who gets referred?
As part of the discharge planning process from the hospital or the emergency room, the physician and the case manager of record will recommend patients for house-call services if they are at risk for readmission. This service will be an option for patient discharge planning just like home health services, nursing home placement and will be provided by Home Physician Care.
