Phase IRehabilitation Phases

The "Healthy Hearts" Cardiac Rehab program is arranged in two phases, building you toward better health.

Phase I

Phase I begins while you are in recovery in the hospital. You will receive information about behavioral changes you should make upon your return home, including safety precautions and directions on appropriate activity levels. You will also begin a low intensity exercise program and learn about the risk factors associated with your heart condition.

Phase IIPhase II

Phase II begins several weeks after your discharge from the hospital. A medical professional will evaluate you to develop a personalized "Healthy Heart" program that meets your needs.

It will include exercise sessions and continuing education classes to help you make changes toward a healthier lifestyle.

Exercise sessions may include walking, biking, weight lifting, and/ or step climbing. A team of medical professional will monitor your heart's response to exercise to ensure safety.

In the course of your program, you will also learn about healthy heart topics, including:

  • Nutrition
  • Stress Management
  • Cardiac Risk Factors
  • Smoking Cessation
  • Cardiac Medications
